We happily complete simple forms same day if they are brought with to the appointment and the child and/or parents has already completed their portion of the form. If the form is received when the patient does not have an appointment, it is complex or requires reviewing the patient record in order to complete, please allow 48 to 72 hours for completion. The doctor will NOT complete forms that have not properly been filled out by the parent or guardian.

No matter what the reason for transferring care, if your child will be leaving our practice, please let us know as soon as possible who will be assuming your child's medical care so that the proper releases can be filled out and records sent. Please allow 3 to 5 business days to get the requested records sent out.

Cornerstone Pediatrics provides care for children until they are 18 years of age or have graduated from college. Ideally, when a teenager or young adult is nearing this transition, we will discuss this it at one of their last well child checks. We are aware that the transition to adulthood can be difficult. We want to make the medical transition as smooth as possible.
